You can order a certificate for your child under the age of 18 years. If your child is between 15 and 18 years of age, it is still the parent(s) who must initiate the request. If your child is more than 18 years old, he/she must make the request.
Please note: If your child is NOT baptised, you must order a Certificate of Personal Data
Your order will be received by your local registrar if you live in Denmark. If you do not live in Denmark – the parish, that you used to belong to, will process your request.

If your child is baptised in the Danish National Church (Folkekirken)
If your child is baptised in the Danish National Church in Denmark, you can choose between two certificates – the Certificate of Personal Data ("Personattest") or a Baptism and Birth Certificate ("Fødsels- og Dåbsattest").
If your child was born abroad, there are certain conditions that must be met before you are entitled to get a certificate.

If you cannot order online with MitID
If you cannot use the self-service, or you need another kind of certificate, you can collect the certificate by attending any church office in Denmark (or municipality in the Southern part of Jutland if you were born in that part of Denmark). Please remember to bring your own photo ID.
Please note: If your child was born in the Southern part of Jutland, you can also contact the municipality where your child was born.

Once you have entered the self-service you have to choose your child and then the specific certificate:
Once you have chosen eg. the Certificate of Personal Data (in Danish: “Personattest”) you have to fill out the following:
“Sprog” (language for the certificate) - choose Danish/English
“Levering” (= Type of delivery).
Choose between:
- “Digital forsendelse” (= a digitally signed certificate that is sent to your digital post)
- “Brevforsendelse” (= the certificate is sent to your postal address)
- "Afhentning” (= delivery in person by attending a church office)
Fill out your E-mail address and phone number, E-mail and “Telefonnummer”
Logo: As a standard the certificate is shown with a logo of the Danish National church (In Danish “Den Danske Folkekirke”. If you do not want the certificate to show the logo, you must remove the mark.
You are now prepared to order, click on the green button “Bestil”.
Read the terms of acceptance and choose “Accepter” (= accept) to proceed. (For more information on the terms and conditions, read more on retsinfo.dk ….)
Once you have accepted you will be met with a notification that confirms your order. The same notification is sent to your email address.
