If you are unable to collect your certificate yourself, you can give another person (over 18 years) power of attorney to collect it on your behalf.
It must be stated in the power of attorney that you (full name, address and civil registration number) authorize (full name, address and civil registration number or birthday) to obtain a certificate on your behalf. The kind of certificate to be collected must be specified, for example Certificate of Personal Data or Baptism and Birth Certificate.
You must sign and date the power of attorney and hand it to/send it by ordinary post to the person who will collect the certificate. Please, do not send it by email.
To collect the certificate the holder of the power of attorney must bring valid photo identification.
The certificate must be collected at the registrar.
